Macmillan Cancer Care Review Template

A Cancer Care Review (CCR) is a conversation between a patient and their GP or Practice Nurse about their cancer journey. It is essential to personalised care and helps patients to talk about their cancer experience and concerns and understand what support is available in their community.

In recognising the important role that primary care plays to support people after a cancer diagnosis changes have been made to QoF in England regarding Cancer Care Reviews.

These changes include:

  • The introduction of a new indicator for a conversation and the offer of support or information within 3 months of a patient receiving a cancer diagnosis

  • The use of a structured CCR template during a CCR within 12 months of a patient receiving a cancer diagnosis (with Macmillan’s national integrated template recommended)

Macmillan has developed a number of resources to support primary care professionals in having these CCR conversations. 

To support the implementation of this we have worked with Macmillan to update and integrate the national Cancer Care Review Templates within Vision +  enabling you to encourage a standardised, holistic, person-centred approach to these conversations.

To access the CCR template in the Vision+ go into Consultation Manager, select the arrow on the right-hand side of the + icon and select ‘clinical templates’.

Macmillan has launched a dedicated Cancer Care Review Webpage which has been designed as the go-to place for further information about what’s required with tools and resources available to support the implementation of this.
