We would like to remind you that confidential information relating to individual identifiers of reporters and patients should not be included within free text fields in reports sent to the MHRA:
‘As you may be aware the MHRA shares Yellow Card data with the World Health Organisation and marketing authorisation holders of the reported suspected medications as part of routine pharmacovigilance activities – further details can be found in our privacy policy. Unfortunately, where confidential information is reported to the MHRA within free text fields there is a possible risk of disclosure of the confidential information to these third parties.’
This includes the Clinical Narrative: Please give details: section which you should complete with details of the reaction, not the patient.
