Yellow Card Confidential Data Reminder

We would like to remind you that confidential information relating to individual identifiers of reporters and patients should not be included within free text fields in reports sent to the MHRA:

As you may be aware the MHRA shares Yellow Card data with the World Health Organisation and marketing authorisation holders of the reported suspected medications as part of routine pharmacovigilance activities – further details can be found in our privacy policy. Unfortunately, where confidential information is reported to the MHRA within free text fields there is a possible risk of disclosure of the confidential information to these third parties.

This includes the Clinical Narrative: Please give details: section which you should complete with details of the reaction, not the patient.
