Clinical Safety Issue - KIS Special Note – Scotland Only

Cegedim has been made aware of an issue where a Key Information Summary (KIS) Special Note recorded for patients registered at Vision GP practices may not be the most recent version of that data. This occurs in the specific scenario where a patient has transferred to a Vision practice and a new Special Note is added to the patient’s KIS by their new practice before their record has transferred from their previous practice via GP2GP. In this case, the new Special Note will be overwritten by the previous Special Note. As a result, the patient’s previously recorded Special Note will be displayed in their KIS instead of the new Special Note. There is a clinical safety aspect to this which may have implications for patient care. Where a viewer of a patient’s KIS Special Note views out-of-date information contained therein and does not have access to the most recent version of that data, there is a potential risk of inappropriate clinical decision making.

As mitigation, we are currently in the process of establishing a development fix that will prevent any further occurrences of this issue and that will be implemented via a future Vision software release. In the meantime, where practicable, we would ask that practices wait until a GP2GP record transfer has fully completed and processed for a newly registered patient before adding a new Special Note to their KIS. We realise that this will not always be possible, for example where information for the patient needs to be shared urgently with another care setting. In this case, please ensure that you have a process in place to review the patient’s KIS Special Note following the completion of a GP2GP transfer until the development fix has been implemented.

To facilitate identification of patients registered at your practice who may be affected by this issue, an ad-hoc Vision search has been produced and is attached as follows:
