We have been working with a team of experienced Clinical Pharmacists to bring you a new Medications Review Pathway.

Developed to help you address your patients' medication needs, the new pathway consists of the following tools:

• Template to help you carry out and record a Structured Medications Review.

• Practice level report to manage your patient's medications lists.

• Central extract to enable review data to be assessed for Clinical Pharmacist teams or Medicines Management teams.

The new Medications Review Pathway is directly accessible within your Vision 3 clinical system for quick and easy use, where all relevant information can be accessed, reviewed and recorded for your patients.
