The Unhealthy Behaviours QI pathway is now available for download by practices in Wales.
The pathway can be downloaded via the Download Web Files tool in Vision+ and includes opportunistic notification alerts, a comprehensive data entry template with links to useful resources, and a set of patient identification reports you can run against your practice population.
